Who We Are, What We Do
|
Sutter Visiting Nurse Association (VNA) & Hospice is one of the largest not-for-profit, community-based home care and hospice agencies in Northern California. Whether you are recovering from or living with illness, our services enable you to receive high-quality health care at home, with comfort and dignity.
The dedicated teams of health care providers at Sutter VNA & Hospice care for more than 22,000 home care patients each year, and provide care for over 350 hospice patients each day in Alameda, Contra Costa, El Dorado, Marin, Placer, Sacramento, San Francisco, San Mateo, Solano, Sonoma, and Yolo Counties.
We care for patients in their place of residence: homes, skilled nursing facilities, and residential care facilities.

In 2006 we will celebrate our centennial, marking 100 years of home care that began with the selfless acts of volunteer nurses who visited sick and injured children following the 1906 San Francisco earthquake and fire.
Sutter VNA & Hospice administrative headquarters are in Emeryville. We provide care from local offices in Emeryville, San Francisco, San Leandro, San Mateo, San Rafael, Santa Rosa, Concord, Sacramento, and Roseville. Each of these local Sutter VNA & Hospice offices has long-standing ties in the community it serves.

Our services include skilled visiting nurses, rehabilitation therapy, home infusion therapy, medical social workers, medical directors, hospice care, home health aides, volunteers, chaplains and bereavement programs, and community health clinics.
We care for people with cancer, AIDS, diabetes, heart and lung diseases, fractures, and strokes, as well as the frail elderly, the chronically ill and disabled, pregnant women, infants, and young adults. Our clinical teams work with your physician to carry out your individual plan of care.

Financial Assistance Available

If you are uninsured, or if you believe your insurance and/or income will not cover the cost of your care, of if there is a change in your insurance, you may qualify for financial assistance. Sutter VNA & Hospice has a financial assistance policy for low-income uninsured patients who may not be able to pay for services. For information about our charity care policy that provides for full or partial assistance, please contact the
local office providing your care.
